I do accept out-of-network benefits and can provide you with a receipt that you can submit to your Flex Spending Plan, HSA, or your insurance provider. Y

ou may still able to use your benefits by filing a reimbursable claim. When you select this option, your insurance carrier will reimburse you for a percentage of the cost of therapy. You will be responsible for paying the full session fee at every session, then submitting a claim to your insurance carrier for partial reimbursement.

Ultimately, it is the client's responsibility to know and understand what services are covered under their individual policy. Please contact your insurance company before your intake (first appointment) to verify your coverage to avoid unexpected expenses. Click here for questions to ask.

Payment

Cash, check, and all major credit cards are accepted for payment. Credit cards will be utilized as primary payment.

DISCLAIMER FOR GOOD FAITH ESTIMATE

Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” (GFE) of expected charges.

The GFE shows the costs of items and services that are reasonably expected for services provided by Hidden Path Counseling, LLC. The estimate is based on information known at the time the estimate was created. It does not take into account any reimbursement that you may receive as a result of out of network benefits.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. You may contact Dr. Jill Mallin to let her know the billed charges are higher than the Good Faith Estimate. You can ask her to update the bill to match the Good Faith Estimate, ask to negotiate the bill, or ask if there is financial assistance available.

Make sure to save a copy or picture of your Good Faith Estimate.
